Primary Care
Primary care covers a broad range of medicine, including preventative medicine with annual checkups, assistance with managing ongoing disease, and treatment of common illnesses and injuries.
Acute Care
Acute care is for illness or injury that comes on suddenly, and can include migraines, flu, infections, colds, allergies, and much more. Our highly-trained health care professionals can help you feel better, fast.
WellMed
HSC Health鈥檚 Center for Older Adults WellMed clinic is a dedicated clinic for Medicare Advantage members. This clinic focuses on preventative care and early detection of illness as well as primary care.
House Calls
When coming to the doctor鈥檚 office isn鈥檛 an option, our expert care can come to you! House calls even include mobile labs and imaging, cognitive testing, and more.
Caregiver Resources

Center for Older Adults Frequently Asked Questions
Most patients start to see a geriatrician when they are 65 years old or older. Our Center for Older Adults team is specially trained to assist with cognitive and physical changes as the patient ages.
Medicaid Advantage Members are welcome to utilize our dedicated clinic for WellMed patients!
Qualifying patients must be 65 years or older and “homebound” due to mobility limitations, problems with transportation, or memory loss with behavioral issues. Patients can be homebound while living:
- At home
- In an independent living facility
- In an assisted-living facility
For referrals, have your insurance information on hand and your discharge summary if recently hospitalized.
A diagnosis of Alzheimer鈥檚 or Dementia will include regularly scheduled meetings with the patient, the care team, a neuropsychologist, and the patient鈥檚 family to discuss what changes to expect over time. These meetings will include practical guidance for day-to-day activities along with medical advice.
