Center for Older Adults Frequently Asked Questions
Most patients start to see a geriatrician when they are 65 years old or older. Our Center for Older Adults team is specially trained to assist with cognitive and physical changes as the patient ages.
Medicaid Advantage Members are welcome to utilize our dedicated clinic for WellMed patients!
Qualifying patients must be 65 years or older and “homebound” due to mobility limitations, problems with transportation, or memory loss with behavioral issues. Patients can be homebound while living:
- At home
- In an independent living facility
- In an assisted-living facility
For referrals, have your insurance information on hand and your discharge summary if recently hospitalized.
A diagnosis of Alzheimer’s or Dementia will include regularly scheduled meetings with the patient, the care team, a neuropsychologist, and the patient’s family to discuss what changes to expect over time. These meetings will include practical guidance for day-to-day activities along with medical advice.
