HSC Qualtrics Access
Qualtrics is a self-service cloud-based online survey, reporting and analysis tool made available to all HSC faculty, staff and students. The intended use of the Qualtrics survey program is to collect information, judgments, or opinions. It should not be used for graded assessments of students. The survey creator will be considered the owner of the data gathered.
Qualtrics must be accessed through this web page to comply with our agreement, log in with your HSC network ID and use HSC templates. HSC team members can activate their Qualtrics accounts by clicking the link above and entering their HSC login credentials to proceed. If you need login assistance, contact the HSC Help Desk.
HSC’s contract with Qualtrics includes technical support and extensive training resources, available at no charge to HSC students and employees through (). Qualtrics can also provide survey panel respondents for a fee. For more information and a quote for panel respondents, contact ().
Qualtrics, Inc. provides extensive training, support (both technical and research), and it can provide survey panel respondents if requested. There would be a charge for Qualtrics to provide panel respondents, but all technical support and training charges are included in the HSC contract.
An HSC-approved survey template is provided. Any changes to the template or additions must be approved by the HSC Qualtrics Administrator (University Assessment and Accreditation), in conjunction with the Marketing & Communications department. Also, all surveys intended for all faculty, staff, or students must be approved by the Office of University Assessment and Accreditation.
Surveys by their nature are inherently research. Any survey that includes the personal information of any “human subject” may require prior review and approval of the North Texas Regional Institutional Review Board (IRB). All surveys and survey owners must comply with IRB rules and procedures before initiation of the project.
